Pretty obvious how the situation is. The sister was Ricardo the whole time and the twin sister is the real patriarch. They probably switched genders to hide. He even mentioned how the patriarch has a smaller build compared to the sister, and this woman he mentioned has a taller build. That just tells it all. Pretty obvious hint from author.
